<p>The Control and Robotics Research Group (TEP 192) of the University of Huelva (UHU), led by the Professor of Systems Engineering and Automation, José Manuel Andújar, is altruistically building protective screens against the coronavirus (COVID-19), which they are donating to Huelva&#8217;s health centres.</p>
<p>For the moment, more than 300 complete kits have been delivered to the groups that had requested them, such as the ICUs at Juan Ramón Jiménez and Infanta Elena Hospitals and the Pediatric Emergency, Pediatrics and Cleaning Services at Juan Ramón, not to mention 061 and the Family Doctor&#8217;s Unit at Isla Cristina, among others. However, the idea of these researchers from Huelva is to continue working to provide protection to all social groups that demand it.</p>
<p>These protection devices are made with 3D printers and consist of screens that cover the entire face, fitted with head supports, which allows for covering a large portion of the face of healthcare professionals, preventing fogging of glasses, as may happen with masks, thus avoiding discomfort when performing their work.</p>
<p>In any case, José Manuel Andújar clarifies that, in general, &#8220;3D printers are not yet conceived or designed for mass production, but to generate prototypes that may later be industrialized. In fact, it takes about two hours to assemble each operation, so an average of 12 computers are made for each printer and day. Of course, the printers are running 24 hours a day. We understand that if, as it seems, companies in Huelva that work with plastic injection have set to work to manufacture protective equipment, this is the best solution to quickly have a large amount of equipment available to professionals who need it. Plastic injection machines can make an enormous amount of daily equipment with as much or more quality than 3D printers&#8221;.</p>
<p>As this UHU professor explains, this solidarity initiative arose spontaneously a month ago, when, following the worsening of the coronavirus crisis, &#8220;we began to have express requests for help from different groups that needed individual protection equipment (IPEs). It was not a coordinated request for help, but individual demands through the members of the Research group (TEP192 Control and Robotics) and the social networks themselves&#8221;.</p>
<p>As a result of these requests, &#8220;we set up a working operation in the research group so that each member could work from home with their own 3D printer to avoid possible contagion and thus comply with the rules of confinement decreed by the government,&#8221; Andújar specifies.</p>
<p>The work has been designed considering issues such as the optimization and commissioning of the printers, from the nozzles and the most suitable plastic, to the appropriate files to obtain the best results, which has allowed the researchers to have the printers operational in a short period of time, working with the same elements and parameters, that is, with similar technical conditions.</p>
<p>Then, each member of the research group has been provided with the necessary material for the manufacture of the screens from their home. Finally, the management of the deliveries has been coordinated from the research group, doing it in a dynamic and quick way, so much so that the equipment has been able to reach its destination in a short period of time, that is to say, not exceeding more than one day, and delivering the screens directly to the applicants.</p>
<p>A proposal that has been very well received by the beneficiary health professionals who even received the delivery of the screens with applause. Furthermore, by way of an anecdote, José Manuel Andújar recalls how, &#8220;given the quality of the equipment being made, certain private companies in the health sector have already contacted us so that we could sell them the equipment we manufacture. However, our work is totally altruistic, never a business, so we supply them to whoever needs them without asking questions&#8221;.</p>
<p>In conclusion, for José Manuel Andújar, initiatives like these show that, &#8220;in the world we live in, a well-organized civil society can greatly contribute to mitigating disasters. I think that when all this happens &#8211; which it will, because however much it rains, it always stops &#8211; our leaders should draw at least a couple of lessons: the first is that we must be prepared for enemies like this one, who will not be the first or the last; and, the second, that civil society, which is well organised, can be the perfect complement, as an auxiliary unit, but absolutely necessary, for the emergency and shock teams that are on the front line, and the assistance teams, which are on the back line. It is absolutely impossible for any government to be 100% prepared for a crisis of these dimensions, just as it would not be for a major earthquake or other catastrophe, but the government can have its society perfectly organized to mitigate both the risks and the consequences.</p>

<p>Members of the Control and Robotics Research Group (TEP 192) join the international community in efforts to produce Charlotte valves using 3D printers as part of their initiatives to address the COVID-19 crisis.  This valve, whose design is available under open license thanks to the company Isinnova in Italy, enables the adaptation of the Easybreath snorkeling mask at the Decathlon sports store and turns it into a device for the treatment of respiratory insufficiency that has already been successfully used in Madrid and Italy.  For its part, Decathlon has blocked the sale of these masks and made them available to various hospitals nationwide.</p>
<p>The initiative of the Huelva research group is based on an express request from 061 for its use in patients affected by Coronavirus.  The mask has been used with good results in mild cases of respiratory failure where intubation is not required.</p>

<p>A team of researchers from the University of Huelva (UHU) has developed a portable electronic respirator that can be very useful in the treatment and cure of patients with COVID-19, by guaranteeing these patients their respiratory needs in a controlled and automatic way. A medical equipment that could assist almost all those affected by the virus who need this type of respiratory resource, which would help to reduce the number of patients who need to be admitted to the ICU, since it could be used to ventilate people even on the ward, avoiding their respiratory deterioration and thus increasing the percentage of survival to the disease.</p>
<p>It is a respirator that was born as a result of an interdisciplinary study, developed by members of the Research group &#8216;Control and Robotics (TEP 192)&#8217;, directed by Professor Dr. José Manuel Andújar, and the group of &#8216;Structure of Materials (FQM 318)&#8217;, directed by Professor Dr. Ismael Martel. Specifically, Prof. Andújar&#8217;s group has been in charge of the design and implementation of the electronics, control systems, instrumentation and communications of the prototype, while Prof. Ismael&#8217;s team has carried out the physical/mechanical part of the respirator, performing the design and implementation of ducts, valves, sensors and integration of all the equipment in the respirator case.</p>
<p>These researchers were joined by three people from outside the University, whose participation has been fundamental to the success of the project, such as José Sánchez, a retired medical physicist from the Juan Ramón Jiménez Hospital, whose experience in electromedicine has been vital for the medical characteristics of the ventilator, as he has been in contact with pneumologists and intensivists from this hospital at all times; Carlos García, a specialist in design and 3D machining; and Ladislao Martínez, an expert in 3D manufacturing with resin.</p>
<p>The developed prototype is of the BPAP (Bilevel Positive Airway Pressure) type, with independent regulation of intrathoracic pressure, i.e. one level during inspiration, IPAP (Inspire Positive Airway Pressure), and another during exhalation, EPAP (Expire Positive Airway Pressure).</p>
<p>As Professor José Manuel Andújar explains, &#8220;the possibility of working with different pressures between EPAP and IPAP is fundamental, as it allows the physician to increase ventilation per minute, thus achieving the airflow demanded by the patient. To this end, the equipment designed has a sophisticated electronic pressure flow controller, which can give 120 litres/minute of air flow, enriched or not with oxygen and at the percentage that the health professional decides&#8221;.</p>
<p>The designed respirator also features a touch screen from which the doctor, in addition to continuously monitoring the patient&#8217;s air pressure and respiratory flow, can interact in a very ergonomic way with the respirator, controlling and selecting parameters such as IPAP pressure (cm H2O); EPAP pressure (cm H2O); I ratio. E.: ratio between the inspiration and expiration time; the trigger value for cycling by flow from expiration to inspiration (l/min); and the trigger value for cycling by time, i.e. the maximum safe time for the device to enter automatic breath control mode. Time-cycling can be used, for example, when the patient is sedated or when the patient cannot breathe on his/her own. In fact, once the trigger values have been selected by the specialist, the ventilator carries out the breathing control automatically, thus ensuring that the patient is ventilated.</p>
<p>In addition to the control knobs, the touch screen also shows the doctor other useful data, such as the selected IPAP and EPAP; the patient&#8217;s breathing rate (BR) or number of breaths per minute; the I-ratio: E ratio selected; the tidal volume or volume inspired by each inspiration (in milliliters); the volume of inspired air in liters/min; the percentage of oxygen supplied to the patient, from 21% in normal atmosphere to that prescribed by the physician; the real-time graph of breathing pressure (cm H2O); the respiratory flow in liters/min; and the respiratory volume in liters.</p>
<p>The developed equipment also has alarms to warn about the maximum and minimum pressure, the minimum breathing flow (liters/m), the maximum FR or if the patient presents apnea, that is, it detects if there is no breathing in a time interval defined by the doctor.</p>
<p>In practice, the device developed has two modes of operation: patient-assisted, where the patient determines the complete breathing cycle; and ventilator-controlled, when the patient is unable to support breathing on his own.</p>
<p>Finally, another of the advantages offered by this design is that the ventilator can be connected to a data network via cable or wifi, or even generate its own wifi network, in the event that no connection is available where it is installed. &#8220;This feature allows, for example, patients in isolation to be monitored without having to enter and exit the room,&#8221; says José Manuel Andújar.</p>

<p>The ventilator was designed by a team from the University of Huelva, led by José Sánchez Segovia, a specialist in Radiophysics at the Juan Ramón Jiménez Hospital, and by University of Huelva professors José Manuel Andújar and Ismael Martel, directors of the &#8216;Control and Robotics (TEP 192) and &#8216;Structure of Matter (FQM 318)&#8217; research groups, respectively, has passed two important tests in view of its implementation as a mechanical ventilation device that can be used in hospitals, either in case it is needed due to the health crisis caused by COVID-19 or in other situations where this equipment is required.</p>
<p>The tests that the device from Huelva has passed, in which professionals from Juan Ramón Jiménez Hospital have also participated, consisted of its application in an artificial lung and in an animal model in a situation of acute respiratory distress, both of which were passed with the maximum clinical guarantees, implying progress towards the test in ICU patients.</p>
<p class="p1">RES(UHU)RGE,</p>
<p>as this device has been named, has differential characteristics with respect to other conventional equipment that have been positively evaluated both in CMAT and by ICU professionals, especially assessing its effectiveness against situations of respiratory distress in animal models with a very satisfactory result according to the report of results of that evaluation, guided by experts in clinical simulation and specialists in intensive care medicine, in particular, Luis Peñas, a collaborator of IAVANTE and a leading expert in mechanical ventilation.</p>
<p>The Technology Transfer Office of the Andalusian Public Health System, which is part of the Progreso y Salud Foundation, is already working on plans for its production, once it has passed the following relevant tests</p>
<p>This new ventilator &#8216;made in Andalusia&#8217; positions the R&amp;D&amp;I of the autonomous community, in which the Regional Government and the universities take an active part, as a system of knowledge generation that during this pandemic is contributing to improve the results of the Andalusian health system, on the basis of the design of equipment, projects, studies and clinical trials that are at the forefront in the country.</p>
<p>In this sense, both the Regional Ministry of Health and Families and the University of Huelva are fully committed to playing a relevant role as an instrument of citizen progress. The RES(UHU)RGE prototype is a good example of excellent, quality, internationally competitive research, which transfers with innovative solutions to help solve the social, technological, economic and health challenges affecting our autonomous community.</p>
<p>Ventilator design</p>
<p>Regarding the design of RES(UHU)RGE, specifically, Professor Andújar&#8217;s group has been in charge of the design and implementation of the electronics, control systems, instrumentation and communications of the prototype, while Professor Martel&#8217;s team has carried out the physical/mechanical part of the respirator, performing the design and implementation of ducts, valves, sensors and integration of the entire equipment in the respirator case. The design of the ventilator has been directed from the clinical perspective by José Sánchez Segovia, radiophysicist, former head of the Radiophysics Service of the Juan Ramón Jiménez Hospital and collaborator of the University of Huelva.</p>
<p>These researchers have also been joined by two people from outside the UHU, whose participation has been fundamental to the success of the project, Carlos García, a specialist in 3D design and machining; and Ladislao Martínez, an expert in 3D resin manufacturing.</p>
<p>In practice, the device developed has two modes of operation: patient-assisted, where the patient determines the entire breathing cycle; and ventilator-controlled, when the patient is unable to support breathing on his own.</p>
<p>Finally, another of the advantages offered by this design is that the ventilator can be connected to a data network via cable or wifi, or even generate its own wifi network, in the event that no connection is available where it is installed. &#8220;This feature allows, for example, patients in isolation to be monitored without having to go in and out of the room,&#8221; says José Manuel Andújar</p>
